Family Voices – What Do You Want From Your Child’s Doctor Survey
This survey was developed by parents from Family Voices organizations in multiple states  to learn from and better understand what other parents want and find helpful from their child’s doctors. This is an anonymous survey, we cannot identify who you are when you complete this survey. Your answers will be combined with answers from other parents into a summary so we can all better understand what is helpful to parents and families and hopefully will improve the healthcare system. This survey contains 18 questions and should take about ten minutes to complete. Thank you for taking the time to fill out the survey and sharing your ideas with us.  If you have any questions or want more information about this survey, please contact Carolyn Allshouse at Carolyn@familyvoicesmn.org.

Esta encuesta fue desarrollada por organizaciones de padres de Family Voices de otros estados para compartir y aprender como el apoyo de los médicos les ha ayudado a entender lo que otros padres quieren para sus hijos.
Esta es una encuesta anónima, por lo tanto, al completar la encuesta no podemos identificar quién es usted.

Measuring Family-Centered Care Survey

Quality of care improves when health care providers listen to families.
When families and health care providers work together, this is called family-centered care.

Family Voices is doing research to develop a way to measure whether health care providers are delivering family-centered care.You can help by taking an online version of a family-centered care assessment tool to test how well it works.

It will take 15 – 20 minutes to complete and will:
– Ask some questions about you and your child. (Nothing that will identify you or your child personally.)
– Ask about how your health care provider works with you and your child. (Your provider also won’t be identified.)

Arc Dental Survey for Adults with Disabilities

Effective July 1, 2012, the Illinois Department of Healthcare and Family Services will be implementing drastic reductions to dental care (also referred to as “oral health care”) for all adults enrolled in Medicaid. We are now conducting a survey regarding how this reduction in services will impact adults with disabilities. We plan to share survey results with the HFS Dental Policy Review Board at their meeting next month.
